MVC 4 Rating System

im quite new to mvc4. Im building an mvc4 rating system.

First, i was created a page to design the rating question as below:

Rating Question Page Creation

Question : [ What Fruit You Like? ] 
Sub Question : Apple ; Orange ; Grapes
Number of Radio Button : [3]
Rating Header : Hate ; Like ; Very Like

Rating Page output :

1. What Food You Like ?

                  Hate     Like      Very Like
Apple             []        []          []
Orange            []        []          []
Grapes            []        []          []

I was store the creation page info into rating db.

How do i show the Rating Page output in MVC 4?
